Care Coordination & Family Advocacy
The "Family Advocate" Role: Serving as the central liaison for all moving parts—doctors, therapists, and family members—so that family can be family rather than case managers.
Continuity of Care Management: Strategically scheduling and supervising a small, dedicated team so the client always sees a familiar, trusted face.
Care Transition Support: Guiding the family through the delicate process of introducing in-home care or transitioning between levels of care.
Crisis Management & Consulting: Providing immediate expertise and techniques when unexpected behaviors or challenges arise.
